For your convenience, we have our new patient form online for you to fill out before coming into the dental office. To make your visit smoother, we also posted our “Material Fact Sheet” online for you to view. The Governor of California has mandated us to provide our patient with information of the different materials used.

Your scheduled appointment time has been reserved specifically for you. We request 24 hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your third appointment without notifying us 24 hours in advance, you may be charged an additional fee.
